Clinical Pharmacy Liaison works closely with patients, providers, and pharmacy staff to ensure smooth, consistent access to medications. This role focuses on supporting patients that are seen by the Clinical Pharmacy team and helping patients overcome barriers to filling their medications to improve patient outcomes and experiences.Overview:
Join our clinical pharmacy team as a Clinical Pharmacy Liaison, working closely with patients, providers, and pharmacy staff to ensure smooth, consistent access to medications. This role focuses on supporting patients who are seen by the Clinical Pharmacy team and helping patients overcome barriers to filling their medications to improve patient outcomes and experiences.
Key Responsibilities:
Support the clinical pharmacy team in daily operationsSchedule and help prepare patients for clinical pharmacist appointmentsAssist with triage of Clinical Pharmacy referralsReceive referrals or warm hand-offs to assist patients with medication accessAssist with prescription processing and billing issues, including prior authorizationsAdvocate for patient access and resolve medication-related access issues. Identify Patient Assistance Programs (PAP) to assist patients with medication access-related issues. This includes all aspects of the PAP, including program enrollment and coordination of patients receiving medication(s).Review adherence reports and contact patients to identify and address barriers to the medication use process. This may include navigating the health care system to connect patients to different members of the health care team.Conduct medication historiesRespond to provider questions within the scope of the technician's practiceHelp manage patients in medication synchronization and adherence programsAct as a liaison across NeighborHealth pharmacy teams and clinical departmentsContribute to pharmacy program projects and quality improvement initiativesSupport overall pharmacy program as neededAbility to work in a functional team and be able to receive direction and instruction from multiple people, including pharmacists and other team members.Required Qualifications:
PTCB Certified Pharmacy TechnicianAdvanced Certified Pharmacy Technician (CPHT-ADV) or obtain within 90 days of hireCertificates must be in the following areas:Regulatory ComplianceBilling and ReimbursementMedication Therapy ManagementMedication HistoryRegistered with the Massachusetts Board of PharmacyMinimum of 3 years of experience in pharmacy, or related fieldProficient in Microsoft Office (Excel, Word, Outlook)Strong communication skillsPreferred:
Fluent in SpanishExperience with Epic and WillowFamiliarity with 340B pricingStrong organizational skills and attention to detailEmpathetic, patient-focused, and a team playerComfortable with multitasking and adapting to new workflowsSelf-driven and able to prioritize tasksDetails:
Position Type: Full-time, on-site. Typical schedule is M-F; however, evening and weekend support may be needed based on the needs of the Pharmacy DepartmentLocation: Neighbor Health CampusHourly Rate:
