Job Details Clinical Pharmacy Specialist - Critical Care Under general direction of the Assistant Director of Pharmacy and the Director of Pharmacy, the Pharmacotherapy Progression Coordinator will work collaboratively with healthcare professionals to maximize positive clinical outcomes in the critical care setting such as optimizing drug therapy to improve patient outcomes, responds or coordinates pharmacist response to resuscitation and time-dependent emergencies, improve the number of preventable adverse drug reactions, mitigate un-necessary use of home medications and ensure appropriate discharge of medications upon transitions of care, support and participate in research, education, and scholarship to advance human health and health care by developing a research portfolio, and educating the next generation of pharmacy practitioners by using the four roles of preceptorship. He/she will participate in organizational performance-improvement efforts related to maximizing effective drug therapy while minimizing adverse drug events and hospital readmissions. He/she will serve as a resource to healthcare professionals and participate on institutional committees as appropriate. As the designated pharmacotherapy subject matter expert specializing in the delivery of pharmacotherapeutic patient care services, he/she will serve as the pharmacy resource to optimize relevant hospital quality metrics the safe and effective use of medications in critically ill patients and rotated throughout the different critical care units at the hospital. Responsibilities: Preparation and Dispensing Accurately and efficiently evaluate, transcribe and/or verify orders for targeted patient populations. Accurately prepare and dispense medications for targeted patient populations. Read, extract and interpret information in patient charts accurately. Monitor drug therapy regimens for contraindications, drug-drug interactions, allergies and appropriateness of drug and dose. Discuss drug therapy regimens with prescribers as appropriate. Critical Care Pharmacotherapy Specialty Practice Collect information about a patient’s present illness, allergies, and past medical, surgical, social, and family histories by using interviews and medical records to ensure safe and effective use of medications Perform a comprehensive reconciliation of a patient’s current and past medications by using data collected from interviews and medical records to determine the pharmacotherapy plan Collect, evaluate, and incorporate relevant data from physical examinations, vital signs, laboratory studies, imaging studies, procedures, advanced critical care monitoring, and other pertinent information by using clinical reasoning to comprehensively assess a patient’s physiological condition and severity of illness Develop therapeutic regimens by using patient-specific data and evidence-based medicine to implement a prioritized pharmacotherapy plan that ensures optimal resource utilization and patient outcomes Collaborate as a member of an interprofessional team by using effective strategies to establish patient-centered and family-centered goals of care Facilitate the administration of medications to patients by assessing availability, route, compatibility, stability, and medication delivery technology to ensure safety, efficacy and timeliness Monitor a patient’s response to therapeutic regimens by using appropriate data in order to evaluate progress toward the goals of care, as needed modify the plan of care, and minimize adverse outcomes Communicate pertinent information by using effective oral and written strategies to ensure continuous and quality care Recommend, implement and monitor operational and clinical pharmacy services consistent with best practices to promote appropriate and cost-efficient medication use Optimize health information technology by using clinical informatics to improve pharmacotherapeutic decision support and minimize patient harm Employ drug information skills by retrieving biomedical literature and evaluating design methodology, statistical analysis, and results to practice evidence-based medicine Contribute to the critical care body of knowledge by participating in research, delivering presentations, publishing, participating in the peer review process, or engaging in other scholarly activities to advance practice Provide education for practicing pharmacists, post-graduate trainees, and students through didactic and experiential methods to promote best practice Mentor pharmacists, post-graduate trainees, and students by using formal and informal methods to promote professional growth Drug Information/Quality Improvement Perform quality improvement activities by reviewing current practices and conducting a needs analysis to enhance the safety and effectiveness of medication use processes Evaluate compliance with organizational policies, accreditation standards, and regulatory requirements by auditing practices to ensure integrity and quality of care Promote the role and optimal use of critical care pharmacists to key stakeholders by documenting performance metrics, quality improvement, safety, and clinical interventions to demonstrate cost effectiveness and to maintain and expand services Develop formulary management strategies through the Pharmacy and Therapeutics Committee and other appropriate channels to improve cost effectiveness, resource utilization, and risk mitigation Collaborate with interprofessional groups by serving on committees and contributing to local, regional, and national initiatives to improve quality of care Provide accurate drug information to professional staff, patients, and families. Document all medication therapy management interventions accurately and completely. Detect and report suspected adverse drug events. Participate in departmental, facility, and/or system-wide quality improvement activities. Collect and maintain data on metrics that may be used to evaluate the impact of pharmacy services on quality measures and patient outcomes Requirements: Education: Graduate of an ACPE-accredited School of Pharmacy with a BS in Pharmacy, an MS in Pharmacy or a Doctor of Pharmacy required Active NYS Pharmacy license required Board Certification in Critical Care Pharmacotherapy (BCCCP) highly preferred Obtain BCCCP within 1 year of employment, required Demonstration of practice experience in one of the following three ways, required: At least four years in Critical Care or Emergency Medicine Pharmacy practice experience within the past seven years Successful completion of ASHP accredited PGY1 pharmacy residency within the past seven years, plus at least two years of Critical Care or Emergency Medicine Pharmacy practice experience Successful completion of ASHP accredited PGY2 pharmacy residency in Critical Care Pharmacy or ASHP accredited PGY2 Emergency Medicine pharmacy within the past seven years Skills: Basic and advanced computer skills; proficient in the use of Microsoft Word, Excel, and PowerPoint. Able to maintain concentration and accuracy despite a busy environment. Outstanding oral and written communication skills. Able to work collaboratively on a multidisciplinary team. Experience: Health-system pharmacy experience required Critical Care pharmacy experience preferred. Salary Range USD $62.72 - USD $84.62 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
