Clinical Program Coordinator RN

Heart Failure Coordinator provides planning and management of all Heart Failure Program clinical initiatives and program development strategies to improve outcomes and achieve business objectives. The position identifies and initiates plans to respond to new clinical and quality program requirements and opportunities for improvement supported by external benchmarking. Provides education, technical expertise and advanced clinical and analytical skills in the management of the heart failure patient population. Coordinates and manages program responsibilities and expectations. Interacts with all levels of the organization, from front-line caregivers to senior management. Collaborates with System/Region for sharing of best practices, reporting methodologies and identification of potential care delivery improvement opportunities.

Required Qualifications:

Bachelor’s Degree in Nursing or Education. Current enrollment in a BSN program may be considered. California Registered Nurse License upon hire. National Provider BLS - American Heart Association upon hire. 2 years Heart Failure Navigator or ICU experience.
