Responsibilities

Working on-site at a major medical center, the Clinical Program Liaison is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services. As a key member of the business development and marketing team, the Clinical Program Liaison develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center.

Qualifications Bachelor’s degree from an accredited School of Social Work; LCSW, MSW or LMSWA minimum of two years’ experience in either Medical-Surgical or Community Health.
