The role you’ll contribute:
Reporting to the Quality and Patient Safety Leadership Team, the Clinical Quality Coordinator is an experienced clinician that assumes a shared accountability for exemplary clinical outcomes and is responsible for providing leadership for planning, coordinating, and conducting organizational quality improvement activities in clinical practice and hospital operational areas across the region. These responsibilities include organizing and managing multiple committees and projects, analysis of clinical performance data, and regular reporting and communications. This position will require real-time and retrospective complex chart reviews, data analysis to determine patterns for action, and identify evidence-based guidelines and facilitate organizational adoption of clinical best practices. The Clinical Quality Coordinator collaborates with team members and physicians on various quality initiatives, as well as tracks, monitors, audits, reports and analyzes clinical and operational data relating to quality.

• Participates in and/or leads hospital and medical staff quality committees, projects, and initiatives to achieved desired outcomes. Assists in the identification of patient care issues, process analysis and redesign, implementation of evidence-based practice and performance measurement.
o Screens complex medical records for quality/outcome indicators as required.
o Recognizes potential quality of care issues and reports them through established systems.
o Schedules and attends meetings, records proceedings, and assists in directing committees to address improvement and educational opportunities.
o Maintains minutes, correspondence, and follow-up activities according to all related policies.
o Gathers, analyzes, and reports data related to performance and achievement of project objectives and timelines. Tracks issues and works to improve processes, efficiency and compliance of best practice standards including electronic documentation standards.
• Concurrent chart review to monitor compliance with publicly reported quality indicators, such as core measures, safety, mortality, readmissions, and other CMS, TJC, Leapfrog, national, state and/or other required measures, coordinates improvement strategies when appropriate, and reports findings at various committees.
• Collects and analyzes data for publicly reported quality indicators and core measures as well as external benchmarking programs and special studies:
o Abstracts data for a variety of databases and identifies performance improvement functions based on data analysis.
o Performs data collection on special studies as requested.
o Inputs data into correct electronic systems.
o Coordinates technical data support and reports preparation for publicly reported measures, improvement activities or other activities.
o Integrates internal and external national or professional benchmark data and summarizes data analysis in a format that facilitates identification and prioritization of improvement initiatives.
o Communicates, reports, and consults publicly reported and/or nationally benchmarked data to various clinical and physician departments throughout the hospital to drive improvements.
• Leads the organization in staying abreast of the CMS quality and penalty programs, TJC changes regarding ORYX measures, Leapfrog Hospital Safety Survey related measures, and other national and state initiatives, recommending changes in processes, as necessary.
o Collaborates with departments and interdisciplinary teams to develop and implement strategies to improve quality outcomes and clinical processes. Recommends and identifies areas of improvement around evidence- based guidelines, from literature, professional standards, and experience and interaction with team members and medical staff.
o Leads, facilitates, consults, and recommends quality improvement initiatives to assist in optimizing performance for publicly reported data in collaboration with hospital leaders and medical staff.
• Collaborates with management and subject matter experts to understand desired performance, assess learning needs, and develop appropriate training solutions for quality improvements.
• Establishes and maintains collaborative relationships with other staff involved in performance improvement activities outside of the quality department, such as Risk Management and other clinical departments.
• Promotes and participates in the professional development of the region with respect to quality improvement.
• Focuses on professional growth by participating in local and national quality professional associations, participating in various webinars (e.g., CMS, TJC, IDPH, Telligen, etc.), and attending educational offerings, as resources permit.
Qualifications
The expertise and experiences you’ll need to succeed:
KNOWLEDGE AND SKILLS REQUIRED:
• Excellent computer skills, including advanced use of Microsoft Word, Excel, and PowerPoint.
• Experience in complex chart reviews, running reports, analyzing data, working with data systems, and ability to manipulate data in multiple modalities such as Excel.
• Experience preparing professional presentations to describe data, results, and analysis to various clinical audiences, including physicians, nursing, pharmacy, and specialty staff.
• Experience in navigating and interpreting the medical literature and identifying articles that summarize best practices.
• Experience in implementing evidence-base guidelines into clinical practice, using process improvement methodologies, data, clinical background, knowledge, and the medical literature.
• Ability to work with an electronic health record.
• Ability to effectively and efficiently coordinate multiple priorities and issues to facilitate data and quality management needs.
• Possess strong communication skills and excellent organizational skills. Ability to maintain and interact in a professional manner.
• Ability to function as part of a team and develop strong multidisciplinary relationships and partnerships.
EDUCATION AND EXPERIENCE REQUIRED:
• Bachelor’s degree from an accredited school/college of nursing, allied health, or healthcare related field.
• Minimum of 5 years’ experience in hospital/acute care, including experience in performance improvement or quality activities.
EDUCATION PREFERRED:
• Master’s degree in healthcare-related field preferred.
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
• Current Illinois state license in field of study as required based on degree (e.g., Registered Nurse in State of IL)
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
• CPHQ certification preferred.
