We are currently seeking a talented Clinical Referral Liaison – PRN to join our dynamic team. Our Clinical Referral Liaisons obtain referrals and complete patient evaluations, within assigned partner hospital locations, to facilitate admissions to Sheltering Arms Institute. They ensure referring physicians, case managers, and other appropriate partnering hospital employees are aware of services or continuum of care. Our Clinical Referral Liaisons act as consultants, marketers, facilitators, collaborators, promoters, educators and practitioners in the course of their daily work.

 

The goal of the Clinical Referral Liaison is to identify candidates who meet program criteria and who will benefit from Sheltering Arms’ services. The process involves developing strong relationships with patients, family members and referral sources. 

 

Duties typically include, but are not limited to: 

Early identification of potential patients Education of patients, family members, and referral sources regarding the effective and efficient utilization of program services and available resources Screening of potential patients through onsite and/or out of area record review including telephone review, clinical physical assessment and interaction with the physicians, nurses, and therapists directly involved in the patient’s care Interviewing family members or significant others to obtain accurate information as to premorbid functional status, personal interests, cultural considerations, and discharge plans affecting the rehabilitation admission. Determination of medical stability, readiness for program participation, and whether services recommended are appropriate, reasonable and necessary. Coordination of a smooth transition from the referring facility to the Sheltering Arms Institute, and recommendations as to appropriate care for candidates who are denied admission. Business development, networking, and marketing to referral sources on an ongoing basis

 

Successful candidates must possess:

Current Virginia clinical license that allows team member the ability to perform patient pre-admission screening. Minimum three years of experience in a health care setting reviewing patients' medical charts to ascertain the medical necessity for services and the appropriateness of level of care. Ability to extract information from medical charts and use clinical judgment. Excellent verbal and written communication skills for safe and effective patient care and to meet documentation standards. Excellent interpersonal communication skills in order to maintain and enhance referring physician relations. Strong computer skills. Mobility to travel to various locations via car; must possess current driver's license. Experience in a rehab setting is preferred.
