SUMMARY

As a Clinical Resource Registered Nurse (RN), you plan, organize, coordinate, evaluate and provide guidance in implementing each patient’s plan of care.

Our Long Term Care Unit (B7) is a 25 bed unit that is considered Med/Surg but functions more holistically as Long-Term Care. Patients on B7 are medically ready and have a difficult discharge process: awaiting placement to a rehab center, pending guardianship, or need to find a new place to call home as their former home is no longer capable of living in. B7 houses mainly a geriatric population and may get behavioral patients on the unit. 

STATUS: Part Time (20 hours per week)

LOCATION: Rochester General Hospital

DEPARTMENT: Inpatient Long Term Care (B7)

SCHEDULE: Day/Evening/Night

ATTRIBUTES

Current New York State Registered Nursing license

Graduation from an accredited School of Nursing; BSN preferred.

For RGH - Minimum 2 to 5 years of acute care experience as a licensed RN.

RESPONSIBILITIES

Development. Develop the plan of care in conjunction with patient, family and others, utilizing community resources and systems; collaborate with health team members to ensure a comprehensive plan of care; use health promotion and health teaching methods appropriate to the patient’s developmental level, learning needs, readiness, ability to learn, language preference and culture

Evaluation. Consider associated risks, benefits, costs, current scientific evidence and clinical expertise when formulating expected outcomes; modify expected outcomes with changes in the status of the patient or the situation; synthesize available data, information and knowledge to identify patterns and variances

Leadership. Teach basic physical assessment skills appropriate for the patient’s chronological and developmental age; guide others in recognizing and assessing the individual cultural needs of the patient/family/significant other and in developing a comprehensive plan of care    

EDUCATION:

BS: Nursing (Required)

LICENSES / CERTIFICATIONS: 

BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
