Clinical Social Worker CM
Description Location: UCHealth Memorial Hospital North, US:CO:Colorado Springs Department: Cancer Service Line Admin FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks) Schedule: Full Time Days. Limited Part Time Days could be available at some locations. Shift: Days Pay: $31.61 - $47.41 / hour. Pay is dependent on applicant's relevant experience Summary: The outpatient LCSW Assesses patients and/or patient's family, caregivers, and/or legal representatives and arranges for needed interventions. Plans interventions to help patients cope with social, emotional, economic, and environmental problems and provides short term treatment planning for mental, emotional and behavioral disorders, conditions and addictions. Responsibilities: • Conducts assessments and ongoing treatment with oncology patients and/or patients' family members/caregivers, and will develop treatment goals and psycho-education to address patients' social, emotional, and economic needs • Provides resources for common concerns like transportation to and from medical appointments, lodging and travel related expenses, financial resources, and information about local, state, and national resources. • Collaborates closely with all members of the multidisciplinary treatment team. Advocates and supports compliance with the patient's treatment plan. • Supports department regulatory requirements and accreditations Skills • Requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision. • Must be able to communicate and collaborate effectively with multidisciplinary team members. • Must be able to utilize advocacy skills on behalf of patients, family members, and caregivers. • Ability to assess patients' progress toward goals, and adjust interventions as appropriate.
