Clinical Social Worker Lead

The Clinical Social Worker Lead oversees social work services provided to patients, using experience to best connect patients to potential care options. This role optimizes patient care, connecting patients to the correct care option for their profile. This position serves as a key figure in a patient's treatment plan in the organization, serving as a bridge figure to ensure that patients receive the correct treatment plan.
Determines which patients to focus clinical social work efforts on, ensuring that patients who need more attention receive the correct Social Worker/Registered Nurse to manage their case while not disregarding patients who may need less attention to effectively utilize the organization's resources for the correct patient.
Analyzes information from both the patient and clinical staff to understand the patient's situation, delegating responsibility based on their analysis to link the patient to the correct area of the organization care or the correct Social Worker/Registered Nurse to manage the patient's case.
Connects patients to care opportunities using vast knowledge of potential outcomes and treatment plans to ensure patients get the highest quality of care that fits their profile.
Plans a care plan for patients to enable the patient to get the treatment that is right for them based on the patient's ability to pay, health condition and other important factors.
Oversees the education of patients, families and staff delegating responsibility to other social workers or in some cases educating patients themselves to assist all parties' understanding of the situation and creating an effective environment.
Serves as a leadership figure to lower stress and tension for patients and staff alike while confirming that patients get the correct care pathway.
Performs other duties as assigned.
EDUCATION:Required: Masters of Social Work from an accredited institution
EXPERIENCE:Required: 3-5 years of recent inpatient clinical social work experience, including 2 years post-LCSW
