General Summary
Provides quality clinical mental health assessments and consultation to providers and supporting clinical care team staff as necessary. They are responsible for triage and management of patients with clinical issues related to their behavioral or psychosocial wellbeing. Additionally, the behaviorist conducts psychosocial assessments, interventions, Transitions of Care (TOC), education, and referrals to inform a holistic treatment plan for patients, in support of Population Health. They are responsible for the management and oversight of a caseload of moderate - high risk Members with complex medical/behavioral /psychosocial needs.

Minimum Education:
Work Experience:
2 years Clinical recent/related experience; care management, home health, medical/surgical behavioral health, and/or critical care. Additional experience related to specialized positions (Oncology, Transplant, Maternity, Behavioral Health) will also be considered. Preferred and 2 years Experience in social work counseling with experience in complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background. Preferred and Less than 1 year Experience in health coaching and motivational interviewing techniques. PreferredLicenses:

Stay current with developments in the field of social work, behavioral health, and mental health treatment. Participate in continuing education, supervision, and peer consultation to maintain licensure and enhance clinical skills. Competency in providing cognitive and behavioral intervention to individuals, families, couples and groups with behavioral health/substance abuse problems. Ability to work effectively in a multidisciplinary team. Excellent communication, organizational, and problem-solving skills. Familiarity with electronic health records (EHR) and documentation standards.
