Position Summary:
This clinical staff pharmacist evaluates and interprets orders; compounds; and dispenses medications according to written orders/prescriptions issued by an authorized prescriber. Identify, prevent, assess, and resolve medication-related problems to ensure a safe and optimal patient outcome. Promote clinically rational drug therapy and sound pharmaceutical care through criteria based protocols.

Position Key Accountabilities:
1. Evaluates, interprets, compounds, and dispenses medications and other pharmaceutical supplies according to written orders/prescriptions issued by an authorized prescriber. Evaluates and clarifies medication orders or prescriptions with prescribers.
2. Identifies, strives to prevent, assesses, and resolves medication related problems to ensure a safe patient outcome.
3. Promotes safe, efficacious, and cost-effective medication therapy by participating in the provision and documentation of clinical pharmacy services and execution of medication utilization initiatives (e.g., IV to PO conversion program, renal dosing).
4. Maintains professional competencies to meet current and future needs of the patients and customers.
5. Oversees technicians in accordance with department policies and procedures. Ensures all patient care provided meets or exceeds all regulations, statutes, and JCAHO standards. Other duties as assigned.
6. Other duties as assigned.
Certification/Skills:
Current pharmacist license in good standing with the Texas State Board of Pharmacy.
Effective verbal and written communication skills are required. Ability to effectively participate on an interdisciplinary team or committee. Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics. Problem solving ability and analytical thinking preferred.
Minimum Education:
Bachelors of Science or Pharm.D. from an ACPE-accredited School of Pharmacy.
Minimum Experience:
One year experience as a pharmacist. Clinical pharmacy experience preferred. Experience using computer applications including word processing, spreadsheets, and pharmacy information systems required. Thorough knowledge of state and federal laws and regulations governing pharmacy services. Proficient in the use and application of new medications and technology.
Physical Requirements:
Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
