Are you passionate about advancing health care practices and improving the quality of life for communities? Join our team to support state-level planning, technical assistance, and health care clinical practice for the Statewide Health Improvement Partnership (SHIP), the Aging and Healthy Communities Unit, and other division initiatives. This is an opportunity to lead efforts in reducing modifiable risk factors for chronic diseases, including dementia, while fostering healthier communities across Minnesota.
In this role, you will collaborate with local public health partners, community-based organizations, tribal governments, and statewide health systems to:
+ Support health care settings to promote healthy eating, breastfeeding support, and active living within health care settings.
+ Increase access to preventive care and chronic disease management for populations at higher risk for chronic diseases, including dementia.
+ Develop and implement strategies to connect clinical practices with community resources for chronic disease prevention and management.
+ Facilitate initiatives that increase age-friendly health system best practices, increase awareness of dementia risk reduction, and encourage healthier lifestyles and better health outcomes.
+ Increase access to population health data and improve the effective use of preventive service reimbursements.
+ Provide training, off technical support and foster regional collaborations to promote health care best practices.
This position is eligible for telework to applicants who reside in Minnesota, Wisconsin, Iowa, North Dakota or South Dakota. Position locations will be determined by the top candidate and will be based out of one of the following MDH district office locations: **St. Paul, St. Cloud, Duluth, Rochester, Mankato, Marshall, Bemidji, Fergus Falls** . Reimbursement for authorized business travel expenses is provided according to bargaining unit guidelines.
**Minimum Qualifications**

Three (3) years of experience in public health, health care systems, community health initiatives, or related areas involving the following responsibilities:
+ Developing and implementing programs that address chronic disease prevention or health improvement strategies.
+ Experience bringing partners from different sectors together to address a common goal.
+ Experience working with diverse populations, such as tribal governments, community-based organizations, and local public health partners.
A related master’s degree may substitute for eighteen (18) months of experience; bachelor’s degree may substitute for twelve (12) months of experience; and associate degree may substitute for six (6) months of experience.
Qualifications to be assessed during the interview phase:
+ Strong understanding of health care systems, public health practices, and chronic disease prevention strategies.
+ Knowledge of health equity principles and strategies to address disparities in health outcomes.
+ Proficiency in data analysis and interpretation related to population health and health care services.
+ Strong interpersonal skills to build and maintain partnerships with stakeholders across sectors.
**Preferred Qualifications**
+ Community Health Education Specialist credentialing (CHES® or MCHES®).
+ Practice Facilitation Certificate or experience facilitating quality improvement projects related to chronic disease prevention and management.
+ Knowledge of metrics, assessment, counseling and referral methods for adults and children.
+ Subject matter expertise or experience with dementia and modifiable risk factors such as physical activity, healthy eating, commercial tobacco cessation, and chronic disease prevention and management.
**Physical Requirements**
Position may require lifting to 30 pounds occasionally.
