JOB DESCRIPTION:
Responsible for assuring Care Coordination and Support (CCS) services are provided in compliance with the Pathways to Success Care Coordination and support organization requirements. Works collaboratively with other service teams to meet the needs of clients assigned via Pathways. Responsible for monitoring effectiveness of services as well as assists with community based child and family team meetings, IMCANS review, crisis prevention and safety planning.

ESSENTIAL DUTIES & RESPONSIBILITIES
Provides regular supervision and monthly development to Care Coordinators.Assures accurate, complete, and timely submission of required documentation and reports per agency policy and/or funding/contract requirements. Provides ongoing case management services for assigned service recipients, including but not limited to assessment, planning, linking, monitoring, referral and advocacy.Presents relevant information for the purpose of assuring quality of care, gaining feedback, and planning changes in provision of personal growth-based services. Identifies and reports all Critical Incidents according to Centerstone policy. Ensures compliance with service contracts, subcontracts, accreditation standards, funding agency expectations, and Corporation policies and procedures.Provides community education as assigned.KNOWLEDGE, SKILLS & ABILITIES
Effectively communicate via written, verbal, in person and virtual methods.
Skill in customer service/experience techniques.Knowledge of role and function of various community resources.Skill in problem-solving.Knowledge of basic service recipient’s dynamics of assigned disability groups.QUALIFICATIONS
Education Level
Master’s degree in human service related field.
Years of Experience
Three years of behavioral health experience.
Supervisory experience preferred.
