What You Will Do to Change Lives
The Clinician provides mental health support services to members served and their families at the program. The Clinician also assists in the coordination of all mental health support services provided by the Team. This includes coordinating and collaborating with team members and outside agencies such as, the Conservator’s Office, Public Guardian’s Office, Probation Department, County Behavioral Health Services, and housing and inpatient providers.
Shift Available:
Full Time Salaried; Monday - Friday; 8:00am - 4:30pm
Expected starting wage range is $68,033.91 - $84,024.91. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)
A Master’s Degree in Social Work or related field. To Make Referrals: This program has a closed referral process and does not take referrals from the community. Referrals must be living in a county-approved long-term care facility and be within 90 days of discharge. Referrals can be made directly from long-term care facilities or County Institutional Case Management via Telecare In-Reach.

Who We Serve: Adults who are homeless with a serious mental illnes who may have a co-occurring substance use disorder, are not connected to outpatient services and are in long-term care facilities (LTC), and will be discharged as a step-down into the community.
