SUMMARY
As a Clinician Navigator, your experience and understanding of chronic illness is key in advocating for patients and providing them with the best healthcare treatment resources and optimal clinical outcomes.

Patient Care. Serve as the patient’s first line of communication throughout the entire treatment program; coordinate multiple and complex technologies as part of the diagnosis and treatment; coordinate appointments and procedures; perform direct patient care, nursing documentation, symptom management, patient and family education and counseling Leadership. Form a collaborative practice among the care management team (i.e. surgeons, nurses, social workers, etc.); provide community outreach and education regarding the program; respond to calls concerning the programCollaboration. Work closely with the advanced practice providers and physicians during patient visits; become aware of the treatment plan, expected outcomes and possible treatment complicationsEDUCATION:

BLS - Basic Life Support - American Heart Association (AHA), NP - Nurse Practitioner Certification - New York State Education Department (NYSED), OT - Occupational Therapist - New York State Education Department (NYSED), PT - Physical Therapist - New York State Education Department (NYSED), RN - Registered Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:
