Atrius Health is part of Optum, a health services company focused on building the leading value-based care system in the country. *SUMMARY* Under general supervision, this Oncology Ambulatory Nurse Case Manager coordinates and manages all aspects of patient care through multiple care delivery systems, acting as the central point of contact for patients, supporting them through transitions of care. Through initial contact and assessment, establishes active partnerships with patients and clinical partnerships with Oncologists, other specialists, PCPs Hematology/Oncology leadership and staff, and internal and external resources. Reports to the Clinical Nurse Manager of Hematology/Oncology/Infusion. _EDUCATION/LICENSES/CERTIFICATIONS_ Graduate of a state-approved school of nursing, Baccalaureate degree preferred (or equivalent education, training or experience). Certified Case Management (CCM) or CCM eligible strongly preferred. Must possess a current unrestricted RN licensure in the Commonwealth of Massachusetts.American Heart Association Basic Life Support (BLS) required. Oncology experience preferred. Experience with an electronic medical records system, EPIC experienced preferred. _SKILLS_ Must exhibit strong critical thinking, problem solving, interpersonal and good clinical assessment skills. Highly motivated and self-directed. Ability to interpret clinical information, assess the implication of treatment and develop and implement a plan of care. Demonstrated ability to prioritize, multitask, and work in a rapidly changing environment with multiple demands. Ability to work effectively in a team.
