SUMMARY OF JOB The Sr. Certified Coder provides high level technical competency and subject matter expertise analyzing physician/provider documented in Complex Outpatient (CO) and/or Inpatient health records to determine the principal diagnosis, secondary diagnoses, principal procedure and secondary procedures. Assigns correct diagnostic and procedural codes consistent with Medical Severity Diagnostic Related Groups (MS-DRG), Present on Admission (POA), Severity of Illness (SOI) and Risk of Mortality (ROM) for inpatient records, and correct Ambulatory Procedure Classification (APC) codes, required modifiers, NCCI and LCD/NCD edits. Utilizes encoder software applications (3M 360, ARMS Comprehensive, HDM, Computer-assisted-coding (CAC), Physician Query, Evidenced Based Code Assignment and communication tools) and all applicable online tools and references in the assignment of International Classification of Diseases-Clinical Modification (ICD-CM) diagnosis and procedure codes, Current Procedural Terminology (CPT)/Healthcare Common Procedure Coding System (HCPCS) procedure codes, MS-DRG, POA, SOI and ROM, APC’s and all required modifiers. Creates Physician Queries consistent with current, compliant coder query practices when clarifying conflicting or ambiguous documentation. Assigns correct code(s) by applying coding guidelines established by: The Centers for Medicare/Medicaid Services (CMS) ICD-CM Official Coding Guidelines for Coding and Reporting, ICD-PCS Official Guidelines for Coding and Reporting. American Hospital Associate (AHA) Coding Clinic for International Classification of Diseases, Clinical Modification. American Medical Association (AMA) CPT Assistant for CPT codes American Health Information Management Association (AHIMA) Standards of Ethical Coding Revenue Excellence and Organizational coding policies and procedures. Abstracts correct patient data elements based on the CMS’s Uniform Hospital Discharge Data Set (UHDDS) guidelines and hospital data abstraction policy and procedure to ensure data reporting integrity in four areas: patient identification, provider information, clinical information of the patient episode of care and discharge disposition. There is an ongoing responsibility for ensuring that the values of Respect, Compassion, Justice, Hospitality, Holitisic Approach, Stewardship, and Collaboration are present in our interactions with one another and in the services we provide RECRUITMENT REQUIREMENTS Completion of an AHIMA-approved coding program or an AAPC-approved coding program, or Associate’s degree in Health Information Management or a related field or an equivalent combination of years of education and experience is required. Bachelor’s degree in Health Information Management (HIM) or related healthcare field is preferred. Experience in Inpatient and Outpatient coding guidelines with less than 3 years’ experience or internal progression from Coding Specialist I. Current experience doing remote coding is a plus. Must be detail oriented and have basic computer skills. Experience with computerized encoders and abstracting systems required LICENSE AND REGULATORY REQUIREMENTS: Certified Coding Specialist (CCS), Certified Procedural Coder (CPC), Registered Health Information Technologist (RHIT), or Registered Health Information Administrator (RHIA).
