Summary :The Coder (Hospital - I) reviews patient medical record documentation and assigns and sequences ICD-10-CM diagnosis codes and CPT/HCPCS procedure codes, when applicable, to outpatient and/or emergency department accounts. Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and in compliance with official coding guidelines and other regulatory requirements. Communicates with providers when needed to obtain complete and specific documentation to accurately assign codes. Follows up and obtains clarification on charge issues. Maintains department standards for timely coding completion.Job Description:KEY ACCOUNTABILITIES:
1. Reviews patient medical records for emergency department and outpatient ancillary services accounts.
2. Assigns ICD-10-CM diagnosis codes to all accounts and CPT/HCPCS procedure codes, when applicable, and sequences as outlined in the ICD Official Coding Guidelines, Uniform Hospital Discharge Data Set (UHDDS), CPT/HCPCS Guidelines, and other regulatory requirements
3. Assigns emergency department professional and facility charges for required accounts.
4. Communicates with providers and other clinical team members to obtain additional information, specificity, or diagnoses needed to accurately complete coding.
QUALIFICATIONS:
• High school diploma or GED
• Diploma/certificate in coding (CCA, CCS, COC, AAPC or RHIT)
• Six months or recent graduate coding program
PHYSICAL DEMANDS:
• Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
• Job classification is not exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
WORK ENVIRONMENT:
• Normally works in climate controlled office environment
• Frequent sitting with movement throughout office space
• Use of computers throughout the work day
• Frequent use of keyboard with repetitive motion of hands, wrists, and fingers
