We are hiring a Collaborative Care Coordinator for our growing team!  As a Collaborative Care Coordinator with ARC,  we offer you the opportunity to work in a dynamic, team-oriented, environment helping individuals overcome substance abuse addiction. The Collaborative Care Coordinator is a vital member of the System of Care.  This position will work alongside the medical/psych providers to provide administrative support to ensure that client care expectations are received based on medical necessity. This will be done through monitoring and communication with providers prior to and/or after visits. This position will perform specimen collection/assessments as requested by providers. This position will review all HLOC send outs and will collaborate with all interdisciplinary teams to deliver individualized
care.
 

Key Responsibilities:

Review medical send-outs and ensure all protocols have been followed.Review medical send-outs and ensure all medications have been received.Review medical send-outs and ensure all follow up appts have been made and transport isscheduled.Review sick visit notes and schedule and complete nursing encounters for follow up.Perform direct tasks requested by the provider.Perform COVID, Flu, Strep, UDS, etc as requested by the provider.Perform PHQ-9 and GAD assessments for Psych as requested.Perform waist circumference as requested by the Psych provider.Perform GeneSight/PGX testing as requested.Ensures all clients have updated vitals in coordination with HCS.Coordinates with HCS in the Med Room to ensure meds are available when the provider hasquestions.Coordinate directly with the provider during provider visits.Review medical related incident reports for accuracy.Precepts non-nursing medical staff when needed.Assists in verifying and scheduling medically necessary off site appointments.Assists in completing Nursing Encounters and Nursing Assessments when needed.Resolves client needs by utilizing multidisciplinary team strategies.Maintains a safe and clean working environment by designing and implementing procedures,rules, and regulations; calling for assistance from other healthcare professionals and DON as needed.Protects clients and employees by developing and interpreting infection-control policies andprotocols.Ensures that antibiotics/anit-viral therapy compliance for those who should be receivingper medical provider orders.Maintains client confidence and protects operations by monitoring confidential informationprocessing.Work closely with the Corporate Medical Operations Team to maintain documentation of client care services by auditing client, facility specific records, and providier with detailed reports as needed.Ensures operation of medical and administrative equipment by verifying emergency equipmentavailability; completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques.Maintains medical supplies inventory in the provider visit area.Maintains a cooperative relationship among health care teams by communicating information;responding to requests; building rapport; participating in team problem-solving methods.Contributes to team effort by accomplishing related results as needed.Maintain a positive, respectful, and professional working relationship with Addiction RecoveryCare Center/Odyssey, Inc.’s staff and consumers alike.Prioritizes personal dependability and maintains excellent attendance.Bring policy/personnel issues to supervision as they arise

Key Experience and Education Needed:

Active license as a Registered Nurse / LPN in the State of Kentucky 3 or more years of experience as a Registered Nurse preferredSubstance abuse experience preferred
