Community Benefit Administrator / Health Equity

With the creation of our new umbrella organization we now have our own identity with a unique payroll, benefits, performance management system, service recognition programs and other common practices across the system. *_Position Summary:_* Provides leadership and oversight to community benefit (CB) activities and compliance, and in the development of a systemwide Anchor Mission (AM). Establishes and maintains collaborative processes with internal constituents and community partners around community benefit capture, reporting and articulation, as well as in the execution of our anchor strategies. Represents HHC to external stakeholders at the local, state, and national levels for both CB and AM, as well as assists in the development, production, and distribution of Community Health Needs Assessments (CHNA) and Community Health Improvement Plans (CHIP). Performs duties in a way that supports the vision and mission of HHC, and the work of providing a fair and just opportunity for all those HHC serves to lead healthy and productive lives and ensuring the communities we serve reach their potential socially, economically, and with respect to health outcomes. *_Position Responsibilities:_* *Key Areas of Responsibility* */Community Benefit (CB):/* Provides leadership to the annual CB Report filings for HHC (in collaboration with tax and finance departments), which will require an understanding of regulatory requirements/compliance. Leads the completion and compliance with annual OHS (Office of Health Strategy) community benefit reporting. Provides leadership to the development and creation of CB strategy, outreach, and reporting. Represents HHC in external CB workgroups and committees including various coalitions and the Connecticut Hospital Association (CHA) and Connecticut and nation-wide CB workgroups. Is a subject matter expert on community benefit reporting on the state and federal level and serves as a resource for internal and external partners. Analyzes the health care environment to identify opportunities for the advancement of community benefit. Provides technical assistance through training, in-services and webinars to ensure the standardization of reporting of programs, activities and investments for staff working in activities that qualify as CB. Facilitates the development, communication and implementation of community benefit policies and procedures. Works with key internal hospital leaders to increase community benefit outcomes and ensure inclusion of community benefit within the organization’s overall strategic plan and financial planning processes. Integrates strategic planning methodologies, benchmarking and performance improvement tools, into CB program planning, priority setting, and evaluation. Advises the Center for Equity and HHC leadership on current and future projected risk with external stakeholders including government policy changes on the local, state, and national levels. Participates in strategic planning efforts, integrated policy analysis and development, and problem resolution. Leverages and supports community benefit expenditures and implementation strategies to align with the organization’s strategic priorities. Collaborates with health system and hospital finance, population health and care coordination departments to implement best practices in CB to improve quality of life and reduce health disparities in the community. Collaborates with Tax, Finance and Legal departments to keep them apprised of CB reporting requirements for financial assistance and other community benefit items, provide expertise on regulatory guidelines and current standards, and collaborate on reporting. Oversees all hospital CBISA databases, handles all communications with Lyon Software and disseminates any information on changes or upgrades to CBISA Coordinators, provide training to Coordinators to keep each hospital’s database current and support reporter onboarding/training as needed. Leads the development and updates on CB education and awareness campaigns for departments and employees. Maintains a working knowledge of applicable federal, state, and local laws to ensure adherence and compliance with community benefit regulations and guidelines. */Community Health Needs Assessment (CHNA):/* Provides leadership to the HHC Community Health Needs Assessment, involving internal and external stakeholders to determine the highest priorities and most critical challenges faced in the community in all five HHC regions in partnership with regional community benefit and engagement leads. Develops an implementation strategy addressing the CHNA community identified needs in all five HHC regions in partnership with regional community benefit and engagement leads. Provides ongoing support during development and continued execution of HHC’s regional community health improvement plans. Supports regions in compliance with HHC’s CB Policy. */Program Management and Administration:/* Partners with key internal hospital and health system clinical leaders and administrators to strengthen community health outreach and identify opportunities to expand community benefit activities. Works with internal team members to ensure health education and prevention programs are evidence-based best practices. Builds internal capacity and awareness among staff and leadership to address identified community health needs. Develops or implements a framework for evaluating health outcomes data to report progress and provide evaluation of community health improvement goals. Analyzes and integrates primary and secondary data from relevant sources to engage leadership and influence strategic planning, advocacy and mission-driven programs. Attends system Community Health & Engagement Team (CHET) meetings on a bi-weekly basis to share and promote best practices with those in community outreach positions across HHC. Supports coordination of meeting content and delivery and provides leadership in driving change and executing system-wide initiatives that flow from HHC’s CHET. */Anchor Strategy and Execution/* Provide support in responding to Healthcare Anchor Network’s (HAN) data collection and analysis requirements. Provide leadership to the development and execution of HHC’s Anchor Strategy. Participate in HAN affinity and peer activities. Bring together the varied projects and associated work streams to drive local impact and social value. Build and maintain partnerships that support better community health, based on community need. Test new ways of thinking about place-based public services in delivering better quality of life within target communities Work with partners to underpin HHC’s Anchor Strategy with strong patient and community engagement and involvement.
