The primary purpose of the Community Coordinator, as a member of the Center of Excellence (COE) team, is to build relationships with individuals with opioid use disorder (OUD) in order to support the patient’s journey towards recovery. They will also perform outreach by meeting with patients in the community.
Among the key duties of the position are the following: Engage patient upon diagnosis of OUD in a clinical setting within the Rutgers University Hospital System (i.e. ED, medical clinic, social services office, etc.) Provide continuity of care by meeting patients in various settings, including, but not limited to, during emergency department visits, addiction treatment appointments, medical appointments, social service appointments, shelter, as well as offering group counseling. Assist qualifying patients in accessing necessary public benefits to further support their wellbeing and recovery. Collaborate with physicians, social workers, nurses and other members of healthcare team serving patients with OUD in providing services. Participate in clinical and programmatic level meetings and ongoing professional development trainings. Position Status Full Time Hours Per Week Daily Work Shift Day Work Arrangement Consistent with the current application of Rutgers Policy 60.3.22 or the applicable provisions of relevant collective negotiations agreements, this position may be eligible for a hybrid work arrangement. Flexible work arrangements are not permanent, are subject to change or discontinuation, and contingent on the employee receiving approval in the FlexWork@RU Application System.
