Community Engagement Program Manager

As the Community Engagement Coordinator, is a Registered Nurse (RN) with experience in community health nursing and a commitment to improving population health. This RN provides clinical supervision for all clinical and non-clinical staff volunteering for community health screening events, participates in patient screening activities (including but not limited to blood pressure, cholesterol or HbA1c screening), provides culturally and linguistically appropriate health education, and links patients to needed follow-up care and/or services.
JOB CRITERIA AND PERFORMANCE STANDARDS:
+ Provides clinical training and signs off on clinical competency for all LGH staff volunteering at community health screening events.
+ Provides clinical supervision at health screening events and ensures that patients receive appropriate recommendations based on screening results.
+ Ensures that all patients participating in health screening events have completed and signed a consent form, and stores consent forms in a manner that protects private health information.
+ Utilizes interpreting equipment (e.g. Pocketalk devices) as needed to effectively communicate with community members.
+ Inspects all clinical equipment between events to make sure devices are in good working order, dispose of expired supplies, and replace equipment and supplies as needed.
+ Follows-up with at-risk participants after community engagement events, with participants permission, to confirm plan for follow-up and assist with clinical linkages.
+ Sources and develops patient-facing health education materials for community engagement events in relevant languages and at relevant literacy/health literacy levels for target populations.
+ Maintains a par of health education supplies and LGH promotional items.
+ Builds and maintains relationships with key community partners, leaders, businesses, faith-based groups, and others to expand opportunities for community engagement.
+ Establishes and maintains an annual calendar of community engagement events that are in alignment with the hospital’s Community Health Needs Assessment (CHNA), CHNA Implementation Plan, and Strategic Plan.
+ Drives the hospital’s Mobile Health Unit (MHU) to community events, maintains the MHU in a clean, working state, and seeks assistance from the appropriate department for any vehicle maintenance needs.
+ Serves as a hospital ambassador, demonstrating compassion and respect in call interactions.
+ Assists the Executive Director with tracking community engagement events and with annual Community Benefits reporting as needed.
+ Maintains patient privacy as required by law and hospital policy.
+ All other duties as assigned.
Requirements:
+ Associate’s Degree or higher in Nursing
+ Current Registered Nurse license in the state of Massachusetts
+ Minimum 2 years of experience in community health, primary care, long-term or acute care
+ Ability to think critically and make sound clinical decisions in the field with minimal supervision
+ Excellent interpersonal and customer service skills
+ Proficiency with Microsoft Word, Excel and Outlook
+ Current Massachusetts driver’s license and clean driving record
+ BLS Certification
