Provides and manages health education programs that help individuals, families, and their communities maximize and maintain healthy lifestyles. Collects and analyzes data and existing reports to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments. May serve as a resource to assist individuals, HCPC (Health Coalition Passaic County) team members and the community in gaining knowledge to improve health outcomes, and may also administer fiscal resources for health education programs. The worker should be someone knowledgeable of the community culture and languages spoken with a goal of providing accurate health based information geared towards reducing identified health disparities and ensure information on community services is made available to individuals. Responsible for the monitoring, planning and developing of community education and outreach programs with community based organizations as determined by HCPC. Interacts effectively and represents HCPC with community partners and professional constituencies. Works closely with the HCPC Executive Director and HCPC team members in executing programs and researching health education resources. Maintains accurate records of all outreach activities, program status, outcomes, and needs.
Work requires a Bachelor degree in health-related field, Community Health or related field experience, and a CHES (Community Health Education Specialist) certification. Knowledge of and a strong desire to work directly with community organizations and experience with project management essential. Bi-lingual in Spanish required.
