Position: Community Health Improvement Specialist

Position Summary: The Community Health Improvement Specialist plays a pivotal role in advancing health programs, project development, implementation, and monitoring system initiatives aimed at enhancing community health. Collaborating closely with the Community Health (CH) division staff, SR Foundation, and various system departments, this role drives data-informed strategies to improve the health and well-being of our service area communities. The specialist is responsible for tracking and evaluating all related initiatives and reports to the System Director of Community Health Policy and Strategy.

Minimum Requirements:

Education:

Bachelor’s degree

Experience:

5 years’ experience in project management, data-driven strategic planning, and/or program evaluation Excellent written and verbal communication skills Ability to lead and/or facilitate meetings, with some public speaking experience Proven ability to complete multiple projects with minimal direction

Other Knowledge, Skills, Abilities:

Proficiency in MS Office (Word, Excel, PowerPoint, Outlook) Strong organizational and time management skills Understanding of Social Determinants of Health, Community, and Population Health initiatives

Preferred Requirements:

Preferred Education:

Master’s degree in Public Health, Healthcare Administration, or a related field

Core Job Responsibilities:

Manage projects for the System Community Health division, including grant-funded initiatives and internal system initiatives Collaborate with the Strategic Services Specialist to present data and outcomes Maintain and update CH content on the Hub and public-facing website Establish and maintain tracking and evaluation of CH projects and community collaboratives Serve on community committees associated with health and well-being throughout the System’s service area Participate in strategic and annual operating plan goal development and tracking Prepare and present progress reports, updates, and findings to communicate the impact and value of ongoing CH initiatives to internal and external groups Coordinate and supervise student interns Perform other duties as assigned
