Position Summary: The Community Health Navigator will help recruit patients for Apicha CHC’s Jackson Heights health center, particularly outreaching to Bangladeshi and/or Filipino communities in Western Queens. This role involves working closely with staff from different departments to make sure patients are smoothly connected to care. This role will conduct outreach to Asian Pacific Islanders, immigrants, and LGBTQ+ communities, and promote Apicha CHC’s services. The role also includes assisting with administrative tasks. The major duties of the position include, but are not limited to: Outreach: + Recruit patients from priority populations + Increase awareness of Apicha CHC services + Educate potential clients about access to care and health services, including adult primary care, pediatrics, dental, women’s health, HIV care, HIV preventive care such as PEP, PrEP, and mental health. + Represent Apicha Community Health Center at various health fairs and other community events + Work closely with other program staff and volunteer community ambassadors + Help recruit volunteers from target populations Education: + · Educate our priority communities on various health and mental health challenges. + · Provide clear, culturally and linguistically appropriate information, especially for those with limited English proficiency. + · Facilitate group meetings in the communities + Client Service: + · Help individuals and families access health services in a culturally and linguistically appropriate way. + · Connect clients to Apicha CHC services such as pediatric care, adult primary care, dental, women’s health, HIV prevention (PrEP, PEP), HIV/STI testing, medical insurance, food assistance, and more. Compliance: + Follow policies and procedures for data entry, reporting, and meeting deadlines. + Enter client and event data as needed and provide updates on tasks or projects. + Participate in program evaluations and quality improvement activities. + Attending required meetings and training sessions. + Work as part of a team across various departments. Qualifications: + High school diploma/GED with 2 years of relevant work experience, or some college with 1 year of work experience. + Strong cultural identification with Asian Pacific Islander (API), immigrant, or LGBTQ+ communities, or lived experience is required. + Ability to communicate in English; fluency in an API language is a plus. + Flexibility to work evenings and weekends as needed. + Ability to reach out and recruit program participants from the target community. + Strong team player with the ability to work independently. + Reliable, punctual, and adaptable. + High energy with excellent interpersonal and communication skills. + Comfortable working with a diverse population in a fast-paced environment. + Ability to manage multiple tasks and work well within the organization. + Comfortable using public transportation. + Proficiency with Microsoft Office (Word, Excel, PowerPoint).
