Job Summary:

Provide care coordination for program-eligible pregnant women to help ensure these women have a medical home for prenatal and post-partum care, practice appropriate pregnancy self-care, get connected to needed social services to remove barriers to care, and learn basic maternal and baby care with the goal of achieving a full term, healthy birth weight birth. Services are clinic and home-based.

Essential Functions:

Works with community providers to identify eligible applicants for the program. Enrolls clients in the program.

Assesses client needs and meets face-to-face a minimum of one time monthly with the client to provide on-going support and assistance. This includes connecting the client to community social services in order to remove barriers to care (i,e., transportation, housing, mental health counseling, etc.), creating and implementing a comprehensive reproductive life plan, and connecting the client to health insurance and a medical home for prenatal care and prevention services.

Educate clients on topics including safe sleep practices, benefits of breastfeeding, and dangers of tobacco use.

Work with client to schedule and attend a postpartum appointment 21-56 days after delivery and remove barriers to care to ensure attendance at this visit.

Maintains and submits accurate and complete electronic and case file records in accordance with timelines.

Participates in supervision on a regular basis and participates in trainings

Performs all other duties as assigned.

Required Minimum Education:

High School Diploma

Licensure/Certifications:

Certified Community Health Worker or in process of becoming certified.

Other Knowledge, Skills and Abilities Required:

Good oral and written communication skills, enjoy working with people, self-starter, culturally sensitive, ability to share educational information and resources in an understandable way, Valid Ohio Driver's License and reliable transportation, computer proficient.

Other Knowledge, Skills and Abilities Preferred:

Knowledge of community resources and issues related to pregnancy
