Your Specific Responsibilities:
The duties and responsibilities of this position includes those functions and tasks of a generalized Community Health Nurse in programs and services related to the health of the community and the individual. Community Health Nursing Services include, but are not limited to: Family Planning, Immunizations, Communicable Disease, Child health. Chronic disease, and other related programs.
Interviews, counsels, educates, obtains health histories, vital signs, administers medications, completes nursing assessments, provides referrals, and follow up care to clients within the clinic and the community. Performs timely and accurate client documentation and coding of services both electronically and manually according to policies and procedures. Assists provider in clinics, conducts screening tests, performs laboratory tests as indicated, collecting specimens in proper tubes, and sending to correct laboratory to be analyzed. Gives and or demonstrates nursing care specific to client's needs. Confers with physicians and other community agencies to provide continuity of care. Tracks and reviews lab results and reports any abnormalities to provider. Adheres to DOH policies for security of client data and security systems.
Administers and counsels on immunizations according to procedure, protocol and ACIP recommendations. Codes services based upon established standards, guidelines, and protocols. Completes vaccine inventory count and submits vaccine orders to immunization supervisor. Reviews temperatures in Verilert system for compliance and signs inspection report in the AM and PM. Reports any discrepancies of temperatures or vaccine inventory immediately to supervisor.
Conducts individual or group demonstrations and/or education on public health programs and community health problems at community events.
Orders, tracks, and maintains an adequate clinical supply of Central Pharmacy medications (nurse issuance and bulk) through PFS. Responsible for inventory accountability and removing all expired medications/vaccines in a timely manner. Responsible to report all discrepancies in Inventory to management.
Participates in In-service and staff development programs to improve job performance and personal development. Assists with orientation and preceptorship of new nurses.
Other duties as assigned by supervisor.
Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties Including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
ON-CALL ASSIGNMENT - This position has been approved in accordance with Section 110.209, Florida Statutes, Chapter 60L-32, Florida Administrative Code, and Collective Bargaining Agreements with the Florida Nurses Association (FNA) and the American Federation of State, County, and Municipal Employees (AFSCME), Florida Council 79. The approved On-Call form has been forwarded to the servicing human resource office. Required Knowledge, Skills, and Abilities:
+ Knowledge of nursing principles, practices and techniques.
+ Knowledge of human anatomy and physiology
+ Ability to assess clients and make sound nursing judgments independently.
+ Ability to safely administer medications and immunizations within scope of nursing licensure.
+ Ability to participate in the planning and implementing of treatment for patients.
+ Ability to effectively interview and assess the health care of clients.
+ Ability to observe patients’ reactions to medications.
+ Ability to draw blood specimens
+ Ability to plan, organize and coordinate work assignments.
+ Ability to establish and maintain effective working relationships, customer service skills, communicate verbally in a courteous and effective manner.
+ Ability to operate a personal computer and accompanying software programs and maintain accurate medical documentation in EMR system .
