InGenesis is seeking a Community Health Outreach Worker to work with our client. In this position, you will work 50% of the time at a mobile clinic and perform street outreach to sheltered and unsheltered homeless populations. Activities include outreach and engagement, development of the care plan, linkage of the patient with resources, and care within the clinic.

Essential Responsibilities:
Advocate for patients and help them achieve their best health. Communicate well with patients, document clearly in the medical record, and communicate with medical providers to advocate for the patient. Conduct outreach and enrollment assessments. Create a care plan that reflects patients' needs. Connect patients to resources. Serve as a liaison between the community and health and social services. Meet departmental review and documentation standards for work assignments. Adhere to the policy and procedures of assigned clinic(s). Build and maintain appropriate relationships. Support a positive work environment and foster teamwork.Minimum Qualifications:
Highschool diploma/AA or BA preferred. At least three (3) years of progressively responsible work experience in a community-based health care service capacity. Bilingual preferred.Physical Demands:
Sitting, walking, and standing associated with a normal medical/clinical office environment. Regular bending and stretching; and lifting. Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard. Traveling to work sites including some exposure to outside elements. Presenting information in front of groups or the public. Reading and writing in order to review records for accuracy. Moderate physical activity. Requires handling of average-weight objects up to 20 pounds or standing and/or walking for more than 4 hours per day.
This position is dependent on contract award.
