As a Community Health Promoter II, you'll create a positive Scripps Health experience for our patients while being responsible for the following:

Identify patients from the emergency department who qualify for Enhanced Case Management Connect patients to Enhanced Case Management at local partner sitesProvide patient support and education which will be offered post discharge to link patients to needed social and medical supportive services.Manage documentation as well as maintaining a list of community contacts and referralsImplement community outreach and facilitatea variety of community-based education.Support a variety ofcommunity-based interventions.Assist to develop educational content.

High school diploma or equivalent.Excellent communication skills. Demonstrate oral and written communication skills by clear and accurate correspondence, including spelling and grammar usage.Demonstrate experience and ability to work independently and as a team player. Ability to set priorities and goals without specific direction. Ability to handle problems, propose solutions, and implement them.1-2 years community-based work experience.Verbal and oral presentation skills.


Preferred Qualifications:

Bilingual in Spanish.Basic typing and computer skills.
