As a Community Health Promoter II, you'll create a positive Scripps Health experience for our patients while being responsible for the following: * Identify patients from the emergency department who qualify for Enhanced Case Management * Connect patients to Enhanced Case Management at local partner sites * Provide patient support and education which will be offered post discharge to link patients to needed social and medical supportive services. * Manage documentation as well as maintaining a list of community contacts and referrals * Implement community outreach and facilitate a variety of community-based education. * Support a variety of community-based interventions. * Assist to develop educational content. *Required Qualifications:* * High school diploma or equivalent. * Excellent communication skills. Demonstrate oral and written communication skills by clear and accurate correspondence, including spelling and grammar usage. * Demonstrate experience and ability to work independently and as a team player. Ability to set priorities and goals without specific direction.
