Community Health Representative
**Job Summary or Objective:** You will provide quality outreach health care services, through health promotion, and disease prevention to American Indians residing in our tribal boundaries through home visitation services, individualize needs assessments, with regular blood pressure and glucose monitoring. You will report to the Community Health Manager or Director. **Location** : Durant **Schedule** : Regular/Full-Time Monday-Friday, 8:00am-4:30pm WITH OCCASIONAL NIGHTS AND WEEKENDS Weekly earned wage access is an option for this position. **Primary Tasks:** 1. You will perform liaison services between Indian communities, tribal programs, and tribal facilities through regular home visitation services to see patient's medical needs are met. 2. Perform basic home health care services by conducting a comprehensive individualize home needs assessments; provide health disease prevention education; and take patient's vitals. CHR's will be on call 24 hours a day due to nature of position. 3. Help schedule medical appointments, transit arrangements, assist with medication refills with the possibility of placing meds into weekly planners and translations of medications instructions to the understanding of patient's cognitive abilities/native language. 4. Provide client health education in the home. Review service plan and performs follow-up to determine quantity and quality of services provided to patient and status of patient's case. 5. You will host monthly Health Outreach Education Awareness Community Meetings, serving as a community liaison for connection of services for the Tribal community and provide Traditional Cooking Classes to the community through outreach activities. 6. Prepare and maintain client files, keeping all in a confidential manner on home visitation with patients by preparation of monthly reports to administration through access of patient's records, documentation of health education, vitals, and community resources information services given and received. 7. Interpret and translate Choctaw language for communication to serve more traditional elderly patients, with knowledge of the Choctaw culture in healing practices, traditions, and religious practices. 8. Offer specialized care by providing patients with wound care, monitoring of any medical in-home equipment for efficiency, and keep up to date on required certifications including CPR, First Aid, and Wound Care. 9. Perform other tasks as assigned. **Job Requirements** + 2-year work experience in a Health-Related Field or CNA + Read, comprehend, and perform tasks, follow oral and written instruction + Knowledge of the Privacy Act, and understand confidentiality practices with relations to Health privacy laws + Knowledge in basic anatomy/physiology, normal medical values, medical terminology, and nutritional and dietary needs disease etiology and resource + Take Vitals, Blood Pressure, Glucose Monitoring, and Knowledge of Normal/Abnormal Readings + Have the desire to exceed job expectations toward meeting the health and social needs of the community through understanding of Choctaw Nation/Community Resources + Understanding of Choctaw language, practices, culture, and religion practices + Must have Valid Driver's Licenses About the Choctaw Nation The Choctaw Nation is the third-largest Indian nation in the United States, with over 200,000 tribal members and more than 11,000 employees.
