Summary :The Outreach Health Specialist, Rural Health is responsible for motivating, guiding, and facilitating action-oriented goals that contribute to improved safety and personal health outcomes through preventive health screening(s) and one-on-one health coaching. Educates and empowers individuals with evidence-based guidelines for disease management, prevention, tobacco cessation, nutrition, exercise, general wellness, and agricultural safety.Job Description:KEY ACCOUNTABILITIES:
Performs patient assessment and preventive health screenings while ensuring cultural understanding.
Conducts patient education through respectful, clear communication.
Ensures effective team interaction to facilitate professional growth and foster collaboration.
Establishes, maintains, and grows client base within the agricultural community.
Assists with fundraising efforts. Networks and maintains relationships with current funders.
Accurately documents visits through current technology, following standard protocols.
Actively participates in professional growth and wellness.
QUALIFICATIONS:
High School diploma or GED preferred
Must be 18 years of age
Three years of experience working in similar jobs
Valid Wisconsin driver license with reliable transportation and the ability to travel regionally
PHYSICAL DEMANDS:
Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance
Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
WORK ENVIRONMENT:
Climate controlled office setting with daily movement throughout the facility or option to work or remotely (secure internet required, no reimbursement)
Conduct visits to farm families and employees to perform preventive health screening
Interaction with department members and other healthcare providers
Flexibility to work a varying schedule as determined by patient needs
