We are looking for a full-time, compassionate Community Health Worker (CHW) to join our NSPG Community Health Team, serving patients across the North Shore area, including Salem, Lynn, and Swampscott, MA. This role is ideal for someone who enjoys building relationships, connecting people with essential resources, and making a meaningful impact on patient health.

As a Community Health Worker, you’ll be an essential link between our primary care teams and high-risk patients, helping them navigate their health journeys. You’ll work a hybrid schedule (Monday–Friday, 8:00 AM–4:30 PM, flexible), with three days on-site at our NSPG practice at 2 Corporation Way, Peabody, MA, and two days remote.
Our Community Health Workers (CHWs) play a vital role in supporting our high-risk patients, many of whom face mental health challenges, substance use disorders, and limited access to healthcare. This is a rewarding opportunity to improve lives, empower patients, and make a lasting impact on the community.
Job Summary
What You’ll Do:•Provide personalized community health support to high-risk patients facing medical and psychosocial challenges.
•Educate patients on how to take their blood pressure properly and support them in improving their blood pressure control.
•Motivate and coach patients in achieving their health goals through, provider practice visits, community visits, and follow-up calls.
•Address Social Determinants of Health (SDOH) by connecting patients to crucial resources such as housing assistance, food programs, and transportation.
•Document patient encounters and collaborate with care teams to ensure comprehensive patient care and advocacy.
•Attend ongoing training and education to enhance service delivery and stay up to date on best practices.
Qualifications
What We’re Looking For:
High School Diploma or GED required.A valid MA driver’s license to meet patients where they are.A passion for helping others, strong emotional intelligence, and the ability to build trust with patients and care teams.

A strong connection to the community – local residents who understand available resources are highly preferred.Bilingual skills in Spanish or Haitian Creole are highly preferred.Prior experience as a community health worker, health coach, or outreach worker is a plus.
