Community Health Worker

Position Summary:

Community Health Worker (CHW) is part of the prevention team to serve as a vital link between participants and healthcare services under the Tennessee Health Resiliency Grant. CHW will ensure coordinated support for each participant by conducting health screenings and assessments, advocating for their healthcare needs, coordinating services, and facilitating educational workshops. This role also involves fostering participant engagement through regular meetings and leading monthly integrated treatment team meetings. This position serves Cocke County only.

 

Essential Job Functions:

Conduct health screenings and comprehensive assessments for program participants. Coordinate individualized service plans to address participants’ unique healthcare and social needs. Advocate on behalf of participants to ensure access to appropriate healthcare and community resources. Facilitate workshops and educational sessions to promote health resiliency and wellness. Meet regularly with participants to provide ongoing support, monitor progress, and adjust care plans as needed. Lead and facilitate monthly integrated treatment team meetings involving healthcare providers, social services, and other stakeholders to promote collaborative care. Maintain accurate and timely documentation of participant interactions, services provided, and outcomes. Collaborate closely with community partners, healthcare providers, and program staff to ensure seamless service delivery. Support data collection and reporting to meet grant requirements. Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation. Collaborate with other WestCare Tennessee team members to promote and meet requirements. Participate in Will engage and complete the required WestCare Tennessee eLearning, and any other program related trainings. Perform other duties assigned by the Project Coordinator/ Director
