Community Health Worker Brooklyn, NY (http://maps.google.com/maps?q=400+Pennsylvania+Ave+Brooklyn+NY+USA+11207-4707) Description The central focus of the Community Health Worker (CHW) CASAC is to identify, bring into treatment and facilitate treatment adherence for individuals within the Brownsville/East New York area who need Substance Use/Behavioral Health services. Essential Functions and Duties: · Work collaboratively with the Youth Empowerment Counselor and Addictions Program Manager, to support the Treatment Adherence goals of the Youth Empowerment Services · Develop referral sources for patients seeking MOUD (Suboxone) Services at BMS via Community Outreach and facilitating publicity. This will entail attending community focus meetings. · Calling/texting clients to facilitate timely attendance in medical and behavioral health appointments. · Communicating with pharmacies to ensure timely MOUD prescription refills. · Build Rapport and Communication with BH, MOUD and adolescent patients to facilitate their treatment adherence. · Precisely and briefly document all services they have rendered for patients in the electronic medical record. Requirements Required Skills/Abilities: · 2 years of demonstrated working with both youth and adults within a social agency or adolescent department of a medical center, must have experience with youth having SUD issues. · Able to establish rapport with adolescents, parents and extra-agency representatives. · Able to attend community meetings, take notes and present with decorum. · Attend 3- Hour Training of the Trainer (TOT) (Currently offered by NYCDOHMH). · Have a working knowledge of East New York, Brownsville, and other surrounding communities. · Ability to prioritize and delegate multiple tasks and projects to ensure each is completed correctly and in a timely manner. · Proficient in Microsoft Office. · Proficient in utilizing Electronic Health Records. · Able to establish and maintain effective and collaborative working relationships and demonstrate compassion, respect, courtesy, and tact when interacting with others. · Actively listen, elicit needed information, and communicate information effectively and accurately. Education and Experience: · Valid OASAS Certified Alcohol and Substance Abuse Counselor Certificate (CASAC · High School Diploma and preferably 2 years of related college credits. · Possess an active CPR certification. · Effective time management and excellent organization and planning skills · Computer literate in Word, Excel, and internet functions. · Bilingual English with Spanish preferred. Physical Requirements: · Ability to attend meetings at BMS clinics as well as venues in the community. · Must be able to lift to 15 pounds at times. · Must be able to traverse all facilities in all types of weather. Salary Description $50,000 annually
