We are looking for a dedicated Community Health Worker to join our OB department, focusing on the care of pregnant and postpartum patients. This role requires Somali language interpretation. As a Community Health Worker, you will apply your unique understanding of Somali culture and language to communicate and educate patients on their reproductive healthcare journey. You will work closely with patients and families from the beginning of their pregnancy through postpartum care, providing education, support, and regular follow-up throughout the pregnancy. Responsibilities will include assisting patients with care coordination, helping navigate the healthcare system, providing gestational diabetes education, breastfeeding education and support, and offering advocacy to eliminate barriers to care.

Associate degree required. Experience in community outreach, preferably with the target audience and culture. Working knowledge of various systems of health care in the community. Community Health Worker certification preferred or Community Health Worker certification obtained within 1 1/2 Yrs Required Basic Life Support (BLS) Current American Heart Association (AHA) Healthcare Provider card per established policy. Required Bachelor's Degree in appropriate subject matter preferred. Prefer experience with culture-related learning principles and effective communication methods. Competent in the skill of Somali language interpretation. A valid Minnesota driver’s license, a driving record in good standing, and the minimum amount of insurance liability coverage may be required.
