Work Location: Atlanta, GA

Description The Community Health Worker supports patients in accessing health related services, including but not limited to, obtaining and ensuring appropriate use of a medical home and overcoming barriers to obtaining needed medical care and social services. Responsible for addressing care gaps specific to patient in timely manner by communicating with patients and guidance through the networks appropriate physicians and care teams Conducts patient screening for social determinants of health to identify needs and follow up through connecting patient with appropriate network partners to address needs. Establishes trusting relationships with patients and their families while providing general support and encouragement. Listens to patients and families feedback about level of care and/or facility conditions; determine appropriate health care team member or department, communicate concerns and/or issues and facilitate resolution. Provide timely feedback to patient. Records all patient care information in care management, EHR or data collection software (training provided). Facilitate patient journey through the network through consistent communication and engagement processes. Communicates with patients, families and other visitors to inform them about policies, protocols, rights and responsibilities; coach staff and providers regarding communication styles and cultural perspectives for best patient interaction potential. Provides patients with appropriate materials to supplement basic health education and encourages patients to be active and engaged in their health and wellness. Executes proactive outreach to the attributed population utilizing registries and other analytical tools.Promotes individual professional growth and development by meeting requirements for mandatory skills competency, build leadership skills for yourself and others in the department.

 

SKILLS AND ABILITIES:

Knowledge of healthcare information technology. Knowledge and understanding of basic medical terminology Knowledge of customer service principles. Knowledge of discharge planning, utilization management, and case management. Knowledge of Microsoft Office (Microsoft Outlook, Excel and PowerPoint) Understanding and knowledge of social determinates of health and health disparities. Effectively manages time and prioritizes activities, yet stays flexible. Skill in communicating effectively with all levels of employees. Skill in understanding basic principles of verbal and non-verbal communication. Skill in mathematical computations. Ability to work productively with other agencies in the community. Ability to be alert and able to concentrate on many different items. Ability to work with people of all social, economic, and cultural backgrounds. Ability to establish and maintain a professional relationship with physicians, supervisors, and other hospital staff. Ability to work calmly and compassionately with patients, families, staff, providers and visitors who may be emotionally distraught, angry or upset. Ability to deal with highly stressful, difficult and emotional situations.

MINIMUM QUALIFICATIONS: High School Diploma, GED or equivalent AND Experience as a Community Health Worker OR Experience as a Medical Assistant OR 2 plus years experience in a patient care or public health setting. Able to work effectively both as a member of a team and with a high degree of independence. Associates Degree or higher in Social Sciences, Healthcare or related field of study, 2 plus years of experience in healthcare setting preferred. Bilingual in Spanish, Vietnamese, Arabic or other language preferred. Additional Details
