As a Community Health Worker in our Complex Care department, you will provide a variety of duties to engage, support, advocate, and educate community members in overall health and wellness. In addition, you will serve as a liaison between health and social services in the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activiaties such as outreach, community education, social support, and coordination. This unique role works in both clinical and community based settings, including client’s homes.
