Community Health Workers (CHWs) are frontline public health workers who have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Under indirect supervision, this individual would work closely with medical providers, primary care teams, and social services agencies to provide care coordination and connection to resources and support to program clients to improve their health and general well-being through education and provision of coordination of care and services. This individual would work in both clinical and community-based settings.
Education and/or Experience
High School Diploma or Equivalent required. A minimum of two years’ direct experience preferred. Experience with navigating local medical and social support systems preferred. Possible bilingual and medical interpreter qualifications preferred based on specific position needs. Possible knowledge and/or experience with certain medical diagnoses preferred based on specific position needs. Must be from the community that is being served or have a familiarity of the community.License/Certification/Registration
This position requires a valid driver’s license and that the employee is insurable by the Avera Health automobile liability insurance carrier. Must have a Community Health Worker (CHW) Training Certificate from a South Dakota approved program or the ability to obtain within 6 months of hire. Must complete the South Dakota Community Health Care Worker Certification within 6 months of hire and maintain certification. A Fingerprint Background Check is required for Community Health Worker (CHW) registration.Work Schedule
Monday-Friday, 8:00am-4:30pm; 80hrs/2wksPay
The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$19.25 - $24.25 per hour
