Summary of Job Duties:The Community Health Worker (CHW) will manage relationships with patients, local community-based organizations, and community members to provide education, resources, and health education.
In addition, Level II CHW will coordinate outreach and engagement activities, using a community-informed approach to address disparities in Arkansas.
Level III CHW will plan, create, and deliver outreach materials and interventions. Level III CHWs will be members of a multidisciplinary outreach and implementation team that uses a community-based participatory research approach to support and educate the community through individual goal planning, case management, and group education and support.
This position requires collaboration with various team members on multiple projects aimed at addressing public health responses and providing culturally appropriate outreach to hard-to-reach populations. Being bilingual in English and Spanish is preferred. The Community Health Worker III (CHW III) will mentor and directly supervise Community Health Workers I (CHW I) or II (CHW II).
Qualifications:Minimum Qualifications:Level I: High School diploma/GEDTwo (2) years of general experience in community outreach, education, case management, and administration experience.Must enroll in a certified CHW Training Program within 45 days of employment
