Summary of Job Duties:THIS POSITION IS IN PINE BLUFF, AR.
The Community Health Worker (CHW) will manage relationships with patients, local community-based organizations, and community members to provide education, resources, and health education. In addition, Level II CHW will coordinate outreach and engagement activities, using a community-informed approach to address disparities in Arkansas. Level III CHW will plan, create, and deliver outreach materials and interventions. Level III CHWs will be a member of a multidisciplinary outreach and implementation team that uses a community-based participatory research approach to support and educate the community through individual goal planning, case management, and group education and support. This position designs works with several team members across varying projects to address public health responses and culturally appropriate outreach for hard-to-reach populations. CHW III will mentor and directly supervise CHW I or CHW II.

Level I:
High School Diploma/GED plus two (2) years of general experience in community outreach, education, case management, and administration experience.Must enroll in a certified CHW Training Program within 45 days of employmentLevel II:
