Assists in the coordination of care and management of caseloads including, conducting patient home visits to evaluate plan of care and on going community service needs. Serves as a resource to staff and others, provides a wide range of supportive office functions for the office based team. **Education And Credentials** **Responsibilities:** AAS - Associate of Applied Science required. BS - Bachelor of Science preferred.
