Care APC clinic is located at Jefferson St Hartford Hospital APC Clinic has over patient visits annually with an average of patients per day We are a busy outpatient ambulatory care clinic setting at a teaching hospital We see patients for preventative annual care as well as patients with a wide range of health problems such as diabetes high cholesterol asthmaCOPD and hypertension Staffing includes attendings APRNs Residents Psychologist Social Worker Nutritionist Pharm D RNs MAs and MAAs APC facilitates an ongoing relationship between patients and clinicians and fosters participation by patients in decision making about their health and their own care The majority of our patients are bilingual with Spanish being their primary language and have health coverage under Medicaid APC provides opportunities for disease prevention and health promotion as well as early detection of problems APC guides patients through the health system including appropriate referrals for services to other health professionals We are recognized as a Patient Centered Medical HomeJob SummaryThe Community Health Worker CHW works to promote maintain and improve the health of underserved residents and their families by providing social support and basic information to promote healthy behaviors This position uses their cultural connectedness to the community to provide information advice and support related to healthy behaviors and access to health and community servicesThis position works as a member of the care team through an integrated approach to care management and community outreachCHWs are connected to the community they serve and are recognized as a ldquocritical linkrdquo to connect underserved residents to beneficial resources and navigate the ever changing healthcare environment Specific activities of a CHW include screening clients in the community for Social Determinants of Health SDOH which includes housing food security transportation utilities personal safety education childcare and financial strain and then taking action to address those needsJob Responsibilitiesmiddot Establish trusting relationships with and act as a liaison for clients to access resources and agencies that assist the client to better manage their healthmiddot Screen residents for the Social Determinants of Health SDOH and then provide clients with the information needed to begin taking action to address those needsmiddot Provide information to clients about the benefits of preventative care to help avoid andor address health conditions including but not limited to blood pressure monitoring and blood sugar monitoring Motivate clients to be proactive about their health while providing general support and encouragementmiddot Provide follow up appointments and referrals at community health screenings for clients in need of connection to a primary care physician or in need of follow up for an abnormal screeningmiddot Assist with data collection and data entry for community screenings community surveys etcmiddot Attend health fairs and other community events to provide information on effective methods of disease prevention and to share resources available in
