Community Health Worker - OPD Adult Primary Care Community Clinic

The Adult Primary Care (APC) clinic is located at 132 Jefferson St., Hartford Hospital. APC Clinic has over 9,000 patient visits annually with an average of 40-50 patients per day. We are a busy outpatient ambulatory care clinic setting at a teaching hospital. We see patients for preventative annual care as well as patients with a wide range of health problems such as diabetes, high cholesterol, asthma/COPD and hypertension. Staffing includes attendings, APRNs, Residents, Psychologist, Social Worker, Nutritionist, Pharm D, RNs, MAs and MAAs. APC facilitates an ongoing relationship between patients and clinicians and fosters participation by patients in decision making about their health and their own care. The majority of our patients are bilingual with Spanish being their primary language and have health coverage under Medicaid. APC provides opportunities for disease prevention and health promotion as well as early detection of problems. APC guides patients through the health system, including appropriate referrals for services to other health professionals. We are recognized as a Patient-Centered Medical Home. *Job Summary:* * The Community Health Worker (CHW) works to promote, maintain, and improve the health of underserved residents and their families by providing social support and basic information to promote healthy behaviors. This position uses their cultural connectedness to the community to provide information, advice, and support related to healthy behaviors and access to health and community services. This position works as a member of the care team through an integrated approach to care management and community outreach. CHWs, are connected to the community they serve and are recognized as a “critical link” to connect underserved residents to beneficial resources and navigate the ever-changing healthcare environment. Specific activities of a CHW include screening clients in the community for Social Determinants of Health (SDOH), which includes housing, food security, transportation, utilities, personal safety, education, childcare, and financial strain and then taking action to address those needs. *Job Responsibilities:* · Establish trusting relationships with, and act as a liaison for clients to access resources and agencies that assist the client to better manage their health. · Screen residents for the Social Determinants of Health (SDOH) and then provide clients with the information needed to begin taking action to address those needs. · Provide information to clients about the benefits of preventative care to help avoid and/or address health conditions including, but not limited to, blood pressure monitoring and blood sugar monitoring. Motivate clients to be proactive about their health, while providing general support and encouragement. · Provide follow-up appointments and referrals at community health screenings for clients in need of connection to a primary care physician or in need of follow-up for an abnormal screening. · Assist with data collection and data entry for community screenings, community surveys, etc. · Attend health fairs and other community events to provide information on effective methods of disease prevention and to share resources available in the Hartford area. ·Travel between various locations within the community to perform duties. · Perform other related duties as required • High School diploma preferred. • Connection to the community being served required. Connection to the community being served preferred. • CT CHW Certification or in process of working towards the certification. • Ability to read, write and understand English. • Bilingual (English and Spanish). • Ability to speak and understand the language of the community being served preferred. • Must be proficient in a variety of computer software applications: Microsoft Word, Excel and other Office applications as necessary. • Ability to quickly build strong relationships with targeted community residents. • Ability to organize, prioritize and plan/schedule work tasks. • Good verbal and written communication skills. • Ability to exercise good judgment related to recommending services. • Must possess the ability to handle sensitive and confidential situations. • Must have knowledge of the community and resources in Hartford area. Long-term resident of Hartford area strongly preferred. • Comfortable with working directly in the community and in non-traditional settings. • Must be able to travel between various locations within the community to perform duties – valid driver’s license and reliable transportation required. • Flexible schedule We take great care of careers.
