Job Summary Community Health Worker will work primarily with specific target populations including but not limited to non-English speaking populations of geographic target areas. CHW will work closely with internal and external stakeholders or partners including but not limited to clinical/medical/dental providers, primary care teams, and other agencies to improve patient care and outcomes. Patient outreach interactions may be telephonic, video/digital, or in-person generally at an office site. We travel to numerous community sites to provide comprehensive medical care and case management. During our weekly clinics we travel throughout the city meeting patients in various locations. This may include travel to tent encampments, hiking through the woods, visiting tent sites under bridges and along riverbanks.
