Hybrid classified roles can be onsite more than the set minimum based on employee preference. + Description + Benefits + Questions What Success Looks Like In This Job Adams County Health Department (https://adamscountyhealthdepartment.org/) (ACHD) is a newly formed organization as of 1/1/2023 and a part of Adams County Government. As of 6/1/2024 it is composed of 6 divisions (Performance and Business Excellence, Strategic Health Initiatives, Epidemiology and Data Science, Nursing, Environmental Health, Nutrition and Family Health), 230 staff, and has an annual budget of roughly 30M. The Strategic Health Initiatives Division (SHI) serves as the primary and expert resource for developing, establishing, and maintaining innovative evidence-informed, community-based public health promotion and disease prevention strategies identified through data analysis and community engagement. Community Health Worker-Temporary ***This is a temporary position with current funding expected to last through April 2026. Funding is subject to Federal changes, and future funding is also actively being pursued. Temporary positions are eligible for paid sick and safety time in compliance with the Colorado Healthy Families and Workplaces Act. Leave is accrued at the rate of one (1) hour for every thirty (30) hours worked. The maximum leave accrual is forty-eight (48) hours. This position is not eligible for any other benefits such as paid holiday leave, Family Medical Leave, health insurance, or retirement benefits. Examples of Duties for Success The Community Health Worker (CHW) is a public health professional, with lived experience, who is an advocate for their community. The CHW works to promote, maintain, and improve the health of their community by providing individualized support to people within their community and fostering relationships with organizations. As part of SHI, the CHW will work closely with community members to provide culturally appropriate resources, health information, and support with applying to or obtaining community resources (healthcare, housing, food, childcare, etc.). The CHW attends community events to listen to and understand community needs, represent Adams County Health Department (ACHD) in the community, build trust between community members and ACHD, promote internal programs and services, and provide COVID-19 education and information. This role will also support data collection in partnership with community members and organizations to assess impacts of COVID-19 on communities and assess preparedness for future potential outbreak events. Community Health Promotion: + Provides informal one on one counseling, goal development, support, and follow up for community members. Informal support can include healthcare system navigation, health education, benefits navigation, etc. + Development and implementation of individual and group health education topics. CHW will utilize adult learning principles to develop health education topics of interest to community, create curriculum for health topic, and provide education to community in an individual or group setting. + Support advocacy efforts for appropriate community/social services resources for community members. COVID-19 Education and Prevention: + Provides COVID-19 educational materials, testing materials, and community resources to address social needs as they relate to COVID-19. Provides guidance to community members and organizations related to COVID-19 guidelines provided by The Centers for Disease Control and Prevention (CDC) and the Colorado Department of Public Health and Environment (CDPHE). + Provides COVID-19 testing kits to community members and community partners as necessary. Demonstrates proper use of testing kits. Provides information on how to access healthcare services following a positive COVID-19 test. Community Engagement and Relationship Building: + Develop and maintain interpersonal relationships with community partners (e.g. healthcare providers, local non-profit organizations, and schools), internal and external customers, and resource providers to create linkages to care and prevention services for Adams County communities, including healthcare services, emergency financial assistance, housing, transportation, and/or food. + Attend and coordinate community events and outreach activities. Serve as primary contact for internal and external partners when organizing and attending events. Gathers and organizes supplies for events and outreach activities. Provides information on available health department programs and resources for community members. + Participate in key informant interviews with identified external stakeholders on methods to support and promote programming. Administrative Duties: + Maintains program-specific documentation. Uses organizational skills to maintain correct forms and tools necessary for documentation. Participates in development of systems to support resource coordination and referral for individuals and organizations. + Enters data into and maintains computer systems or databases. Proofs data entry and corrects errors. Runs standard and ad hoc reports as required.? + Provides administrative support as assigned for the program. Handles confidential matters, including healthcare information protected by HIPAA, and collects information as requested for the use of the program staff. Filing, typing, and answering phones in support of daily program functions. Maintains files and records.? Additional Duties: + Travel may be required to any ACHD clinic or outreach event as needed for efficient program operations. Travel may be determined on the day of work, based on program needs. The CHS may use their personal vehicle or an Adams County vehicle for events. + Ability to work evenings and weekends as needed. + Ability to work community outreach events, including tabling, teaching, and/or mobile services. + Performs other duties as assigned and required.? Working Conditions and Physical Requirements: + Work is conducted both indoors and outside at events. Routine exposures and hazards of a typical office environment. Hours, Location, Travel: + Hours: Typical work hours are 8am-5pm but will vary based on the needs of the organization. + Location: This position will be mostly in person, with occasional remote work options. Most office work will be based in the health department location in Westminster, CO. Travel: The incumbent of this position should expect to travel between 50%-75% of the time between ACHD locations and other sites within Adams County. Mileage reimbursement or use of an Adams County vehicle are available for this position. Language Pay Incentive is available for employees who utilize their skill in Spanish or ASL 30% of the time or more and pass a basic fluency test.
