As the Community Impact Director, you will make an extraordinary impact by working with staff to advance strategic region priorities for the Southeast Region. The Director will drive the execution of health impact goals within the market by focusing in the areas of hypertension, cholesterol, nutrition, obesity, and physical activity with a focus on diverse communities. This is an office and community-based position based within the St. Petersburg market, reporting to our St. Petersburg office. The ideal candidate must reside within the comfortable market limits of the position to navigate the market and territory, ideally within Pinellas or Hillsboro Counties. Be Valued™ at the American Heart Association by following us on LinkedIn, Instagram, Facebook, X (formerly Twitter), and at heart.jobs. **Responsibilities** + Drive market health assessments, coordinate and/or lead community collaborations, build and implement plans for policy, system, and environmental change driving toward opportunities for health impact in both the clinical and community setting. + Build and implement a cross-functional strategy to advance health equity for all by producing measurable outcomes for improvement American Heart Association priorities: improving hypertension, diabetes and chronic disease management, increasing nutrition security, ending vaping and tobacco use and increasing community readiness. + Work with healthcare organizations to ensure best practices for treatment of patients through education and quality improvement initiatives and programs including but not limited to: Target: BP™, Target: Type 2 Diabetes and Check. Change. Control. Cholesterol™. Consult with clinical staff for data entry, data reporting, compliance issues, and strategies for improved patient outcomes. + Work with internal staff partners to ensure population health strategies are integrated into development, communications and marketing, advocacy, and healthcare quality improvement activities. + Collaborate with development team and volunteers to identify, champion, and secure program funding, including sponsorship (cash and in-kind) for relevant programs, campaigns, and population health impact opportunities. + Recruit, train and lead volunteers and strategic community alliances to achieve priority community, clinical and collective impact goals. + Identify and assist in recruiting diverse volunteers to serve in leadership roles on market board of directors and event executive leadership teams. **Qualifications** + Bachelor’s degree or equivalent work experience. + 2+ years of project management experience in similar capacity. + Ability to analyze data, recognize trends and recommend initiatives to improve outcomes. + Knowledge of the social determinants of health as it relates to behavior modification through self-care activities and policy/system-wide changes is preferred. + General knowledge of fundraising practices and/or corporate social impact sponsorship development is preferred. + Effective interpersonal skills with demonstrated ability to develop collaborative working relationships internally and externally. + Proficient in Microsoft Office including Excel, Outlook, PowerPoint, and Word. + Ability to travel occasionally within the region approximately 70% of the time. **Compensation & Benefits** The American Heart Association invests in its people.
