Community Living Senior Program Manager

Fraser is hiring a Senior Program Manager to provide leadership, oversight, and supervision for assigned Community Living Sites and Services. This role is responsible for driving continuous quality improvement in service development and program management while ensuring compliance with regulations and alignment with Fraser’s vision, policies, and budgetary guidelines.

Job duties:                                                                                                                                              

Strategically grow the Community Living programs by promoting the program to consumers, legal representatives, county staff, and other referral sources. Actively manage resident admissions to assure Fraser resources are optimally utilized, conduct marketing and outreach as appropriate. Oversee completion and ongoing management of the Rate Management System for persons served (RMS), including completing service agreement requests to lead agencies and subsequent follow-up to ensure timely billing practices. Oversee routine operations for Community Living sites and supervise on-site as necessary. Respond to emergencies immediately and provide direction and support to all staff and individuals. Implement staffing patterns based on the needs of individuals, maintaining the budget for staffing. Assist in collaboration with property management to ensure all Community Living sites are well maintained. Develop, maintain, and successfully manage implementation of the annual budget for the Supportive Living division. Work with the Director of Community Living to discuss and modify long and short-range strategies to support Supportive Living. Establish and maintain strong working relationships with referral, funding and regulatory sources.

Location, Schedule & Pay: 

Bloomington, MN  This position will be able to work some from home but expectations for this role are to be on site 2 or more days per week.  Monday through Friday from 8 am -4  pm or 9 am -5 pm; on call responsibilities The starting pay for this role is $67,000 to 72,000 annually depending on qualifications 

What qualifications are required?

BA / BS in human services or a related field preferred. Designated Manager as defined by 245D regulations. Ability to pass certification course to administer medications and treatments (within 60 days of hire). At least three to five (3-5) years of supervisory and managerial experience, working with people who have intellectual/developmental disabilities or related conditions. Ability to meet the program management and oversight criteria outlined in MN statute 245D.3. Proficiency in establishing and organizing new Supportive Living sites and services required.
