Community Outreach Coordinator - Sinai Hospital - SHARP

Summary

JOB SUMMARY:   The Community Wellness Coordinator oversees and coordinates LifeBridge Health health-related programming aimed at promoting LifeBridge Health and growing the market share for their members and the surrounding communities. The Community Outreach Coordinator oversees, and coordinates Sinai Hospital Addictions Recovery Program (SHARP)-related programming aimed at promoting SHARP and growing the census. This includes coordinating intakes into the program and acting as team lead in the program. Develops program strategies and goals that promote LifeBridge Health branding within the communities and are aligned with LifeBridge Health strategic goals. Identifies and develops community stakeholder relationships. develops a full calendar of engagement activities and communications to ensure robust and frequent communication leveraging the broader tools of the LifeBridge Health marketing team. Manages wellness and educational programs aimed at building awareness of LifeBridge Health brand. Plans, coordinates and executes surveys, events, lectures and screenings using LifeBridge Health resources. Provides Departmental reports as required. Gather participant information from events. Track any potential patient referrals to LifeBridge Health providers. Prepare monthly report for Operations Director, Executive Director, and LifeBridge Health Marketing Director on program status. Collaborate & Coordinate with LifeBridge Health marketing departments to implement marketing for programs. Coordinates and works with the Marketing department's Community Outreach Coordinator on all LBH programs and events. Works with outreach programs across the system to coordinate efforts throughout the LifeBridge Health service area. Serves as a liaison linking potential patients with LifeBridge Health resources and potential guests with programs and services. Works with community stakeholders to identify and implement program and collaboration opportunities. Provides outreach and communicates with local community resources. Coordinates to increase joint brand access points within the community. Serves as a representative and advocate for the services of LifeBridge Health. Identifies and develops community stakeholder relationships. Provides departmental reports as required. Gather participant information from events. Track potential referrals to Sinai Hospital Addictions Recovery Program (SHARP). Serves as a liaison linking potential patients with SHARP resources. Works with community stakeholders to identify and implement program and collaboration opportunities. Provides outreach and communicates with local community resources. Serves as a representative and advocate for the services at LifeBridge Health. Manages the intake process for SHARP including responding to all referrals, screening all referrals for eligibility for services (consulting with the members of the SHARP medical and/or leadership team when needed), and scheduling and coordinating intakes. Refers all applications who are not eligible or interested in services to other appropriate providers. Ensures that the admission process moves as smoothly as possible. Updates and maintains the intake spreadsheet. Oversees the clinical and administrative functions of the program. Provides oversight of the milieu in the clinic and Page 1 of 3 | July 15, 2025 monitors/intervenes when patient or staff issues arise. Review new intake charts and provides feedback to staff when needed. Assigns new cases to counselors. Functions as primary counselor providing direct treatment using individual, group, or family counseling and crisis intervention techniques as clinically indicated. Documents all work as dictated by department and program policies, regulatory agencies and insurance carriers. Maintains and updates patient records and documents information regarding patients' status in treatment, changes in treatment plan or recommendations in accordance with program policy, regulatory and accreditation standards, e.g., TJC, COMAR, etc.

REQUIREMENTS:

Minimum of 3 years of experience required; 5+ years of experience preferred. Bachelor's Degree preferred with major in Exercise, Science, Fitness, Health Education or related field or a marketing event background. Prior event planning preferred. Preferred license as one of the following: Certified Associate Counselor - Alcohol and Drug License - Current Maryland license or eligibility to obtain Maryland license Certified Supervised Counselor - Alcohol and Drug Licensed Clinical Alcohol and Drug Counselor - Current Maryland license or eligibility to obtain Maryland license Licensed Clinical Professional Counselor - Current Maryland license or eligibility to obtain Maryland license Licensed Certified Social Worker - LCSW - Current Maryland license or eligibility to obtain Maryland license Licensed Graduate Alcohol and Drug Counselor - Current Maryland license or eligibility to obtain Maryland license Licensed Graduate Professional Counselor - Current Maryland license or eligibility to obtain Maryland license
