Community Outreach Peer Recovery Coach - FT Day/Evening Rotator
The Overdose Survivor’s Outreach Project (OSOP) Community Peer Recovery Coach is an integral part of the OSOP Peer Recovery team that is designated to conduct outreach and engagement to survivors of an opiate overdose after discharge and referral from the Emergency Department (ED). This individual provides non-clinical services intended to aid patients in establishing recovery and reducing the risks associated with a subsequent overdose. The OSOP Peer Recovery Coach will assist in completing referrals to recovery support and substance abuse treatment services as well as developing service plans to promote successful linkage to referred services. Services will primarily be provided in the in the community, however the coach may meet patients in the ED to assure successful continuity in care.

Education

High School Diploma or G.E.D. is required

Experience

The candidate must be actively engaged in his/her own recovery with a minimum of two years demonstrated personal recovery experience from alcohol and/or drug use.

Skills

Licensures, Certifications

obtains certification through the Maryland Addiction Professional Board within 18 months of hire and participates in continuing education programs

Working Conditions

Works primarily in the community and in the ED with some exposure to noise, dust or temperature.

Principal Duties and Responsibilities

Conducts street outreach to locate and engage with referred OSOP patients from hospital EDsConducts screenings to understand the patient’s history of drug and/or alcohol use, treatment history, social service, and other recovery support needs and motivation to change behaviorFacilitates patient’s readiness for change and uses motivational interviewing techniques and other communication skills to support and encourage the patient to plan for reduction or elimination of drug and alcohol useAssists patients in setting personal recovery goalsWorks with healthcare team and patient to identify community supports and treatment services to promote recoverySupports patients and families to understand how to access community resourcesAssists patients in linking to community supports and treatment services, including helping secure transportation and other resourcesActs as a peer support after patient is discharged from the hospital; this involves face-to-face and phone engagement with patients in various community settingsProvides family members with recovery support materialsProvides education on risks of overdose and assures that patient and significant others have and are trained to use naloxoneCompletes required reports and other necessary documentation accurately
